The Partnership seeks support for the following:

Coverage

  • Personnel, computers and marketing resources for FQHCs and safety net clinics to reach out and enroll the estimated 20% of Milwaukee’s vulnerable population who are eligible for government programs.

Access

  • Primary Care Services at FQHCs
    • Capital for facility expansion
    • General support for care of the uninsured
  • Medication Access
    • Personnel for project coordination and Prescription Assistance Programs training
    • Medication assistance software and subscription fees for discount purchasing programs

Care Coordination

  • Care Management and Navigation
    • Case managers within FQHCs and safety net clinics to assist with intake and retention of ‘un-established’ patients
    • Community Health Workers (navigators, social work assistants and health coaches) to assist with outreach, enrollment and promotion of medical /health homes
  • Health Information Technology
    • Expanded functionality of the ED Linking Project to include diagnostic information (lab results, images, etc.), medication information and patient care plans.
