The Partnership seeks support for the following:
Coverage
- Personnel, computers and marketing resources for FQHCs and safety net clinics to reach out and enroll the estimated 20% of Milwaukee’s vulnerable population who are eligible for government programs.
Access
- Primary Care Services at FQHCs
- Capital for facility expansion
- General support for care of the uninsured
- Medication Access
- Personnel for project coordination and Prescription Assistance Programs training
- Medication assistance software and subscription fees for discount purchasing programs
Care Coordination
- Care Management and Navigation
- Case managers within FQHCs and safety net clinics to assist with intake and retention of ‘un-established’ patients
- Community Health Workers (navigators, social work assistants and health coaches) to assist with outreach, enrollment and promotion of medical /health homes
- Health Information Technology
- Expanded functionality of the ED Linking Project to include diagnostic information (lab results, images, etc.), medication information and patient care plans.
