In addition to enhanced care coordination between emergency departments and primary care medical homes, the Partnership seeks to grow care navigation strategies and improve care management for individuals with chronic health conditions.

We focus on two areas:

• Emergency Department Care Coordination: Linking emergency departments and medical homes.
• Shared Community Health Needs Assessment: Learning more about what needed for better health.
